Personal statement (no longer than three pages) reflecting your educational life mission and demonstrating ongoing contributions to psychiatric education
Current curriculum vitae

Qualifications for Consideration:

AAP Fellow Designation:

Five (5) years of active AAP membership
Ongoing, active participation in psychiatric education
Attended at least three (3) annual meetings during applicant's membership with one (1) meeting attended in the last three (3) years
Membership in minimum one (1) or more AAP committee(s) for at least three (3) years

AAP Distinguished Fellow Designation:

Ten (10) years of active AAP membership
Ongoing,active participation in psychiatric education
Attended at least five (5) annual meetings during applicant's membership with one (1) meeting attended in the last three (3) years
Membership in minimum of one (1) or more AAP committee(s) for at least seven (7) years

Applicants for AAP Fellow Designation will complete pages 1 and 2.

Applicants for AAP Distinguished Fellow Designation will complete pages 1 and 3.
